No Objection Certificate (NOC) for Kidney/Liver Transplantation




The following documents should be submitted to the Mission for issuance of NOC for Kidney/Liver Transplantation:  



1. 01 (one) copy of duly filled in application form (Downloadable from Website)


2. Request Letter from the Hospital


3. Declaration Form from the Hospital (Available at the Website)


4. Affidavit from Patient, Donor and their guardians (Must be attested by the Ministry of Foreign Affairs in Dhaka) 


5. If the Guardians of the Donor/Recipient (if unmarried, then Parents)/ (if married, then Spouse) is dead and Donor/Recipient wants to use another person as Guardian, then death certificates need to be submitted (Must be attested by the Ministry of Foreign Affairs in Dhaka) 


6.  Family Tree/Certificate of the relationship between the donor and the patient (Must be attested by the Ministry of Foreign Affairs in Dhaka) 


7.  Passport & Visa copies of the patient, donor and guardians (Must be attested by the Ministry of Foreign Affairs in Dhaka) 


8. Donor’s Declaration in Person at the Mission


9. Doctor’s Certificate outlining the need for transplantation and fitness of the patient and Donor


10. Filled in up Form-21 (The Mission will endorse only the documents attested by Ministry of Foreign Affairs in Dhaka)


11. Fees: NEFT of Rs. 440 in favour of Bangladesh Deputy Commission, Chennai (For Bank Details of Bangladesh Deputy Commission, Chennai please go to the Bank Details to Pay Consular Fees Icon)
