1. 01 (one) copy of duly filled in Mission’s Leave Application Form (Downloadable from Website)

2. Application to Controlling Authority

3. Passport & Visa copy of the Applicant

4. Passport & Visa copy of the Patient (if Applicant is Attendant) 

5. Recommendation letter from Hospital

6. Previous Government Order (GO) of Leave

7. Fees: No Fees Applicable
